Your medical records are important to you. We want to make it as simple as possible to get them, but we're required by law to obtain certain written information. Please follow the steps below.
To request a copy of your birth certificate
Birth certificates are not issued by the hospital, but by the state government:
To have copies of your Legacy medical or billing records mailed to you personally
If you're the patient or the parent/legal guardian of a minor patient, complete this Patient Access Form form and mail or fax it to the Release of Information Department at the Legacy facility where you received your services. (Information for Legacy Medical Centers is below; for primary care clinics please contact the clinic directly.)
To have copies sent to a non-Legacy health care provider, insurance company, etc.
Complete this Authorization form and mail or fax it to the Legacy facility where you received your services. (Information for Legacy Medical Centers is below; for primary care clinics please contact the clinic directly.) If you're asking for the records of another person you need to include proof that you represent them, so be sure to read the instructions carefully.
