Online Payment Help
We are pleased to offer our patients the ability to pay their bills online with either a major credit card or an electronic withdrawal from a checking or savings account. Our online payment portal is through U.S. Bank and Instamed, the industry leading healthcare payments network and platform. All data is encrypted to protect your confidential information and security is tested daily.
Follow the steps outlined below to make on online payment. See Frequently Asked Questions for additional information. If you have questions not covered in the FAQ, please contact Customer Service.
Step 1 - Select the Link (Legacy website bill pay page) On the bill pay page, select the select the link that corresponds with the name and address in the upper left-hand corner of your statement. Here's an example of how the address looks on a Legacy statement:

When you click the facility name, a new window will open that contains the payment form for that facility. Check to be sure the facility name and address on the payment form matches the name and address on your statement.
Step 2 - Select the Payment Type (Instamed website, US Bank Payment Form) On the payment form, select the method of payment you prefer: Credit Card (VISA, MasterCard, Discover or American Express) or Bank Account (you may use a savings or checking account).
Step 3 - Enter the Account Information Enter the account number (found in the upper right-hand corner of your statement). Here's an example of what the account number field looks like on a Legacy statement:

The statements for Legacy's hospitals include initials prior to the account number (Example: for Emanuel Hospital, the account number would look like EH 123456). Please enter the number portion of the account only (Example: 123456).

Step 4 - Enter the Patient Information Enter the patient's last name, first name and birth date (the date needs to be in the format MM/DD/YYYY ; Example: 02/22/1983 ).
Step 5 - Enter Your Payment Information If you chose "Credit Card" as the payment type, you will have these fields to complete:

Please be sure to enter the 16-digit card number with no spaces or dashes between the numbers. For "Amount" you may enter the balance due on your account, or if necessary, you may make a partial payment in any amount.
If you chose "Bank Account" as the payment type, you will have these fields to complete:
Your checking account number and bank routing number can be found on the bottom of your check. The first 9 numbers from the left at the bottom of your check are the routing numbers. This number is always 9 digits. The numbers on the right end of the string of numbers (usually 4 digits) are your check number. The numbers in between are your account number (see the example below). If there are extra zeroes before or after your account number, please include them as part of your account number.

If you prefer to use a savings account, you will need to request the routing number from your financial institution.
For "Amount" you may enter the balance due on your account, or if necessary, you may make a partial payment in any amount.
Step 6 - Enter your billing address or personal information If you chose "Credit Card" as the payment type, you will have these fields to complete:

Please enter the billing address for the card you are using.
If you chose "Bank Account" as the payment type, you will have these fields to complete:

Please enter the name, state of residence and telephone number of the owner of the bank account.
When you are satisfied the information you provided is correct, click the "Next" button at the bottom of the page to continue. To protect your security, your payment account is not stored in the computer - when you click "next" only the last 4 digits of your credit card or bank account number will appear on the next screen. If you made a mistake, or decide to not make an online payment, you may click "Clear" to erase your information from all fields.

Step 7 - Confirmation Screen This screen provides a summary of your payment. If the information is incorrect (or you change your mind), click "Cancel" and you will be returned to the U.S. Bank payment form. If the payment information is correct, click "Confirm." The next screen takes a minute to load, so please be patient.
Step 8 - Payment Receipt Screen This screen provides a printable receipt. (As above, this receipt includes only the last 4 digits of your payment account.) At the top right of the screen, you have the option of entering your email address to have the receipt emailed to you. At the bottom of the screen, you have buttons that allow you to:
Print - Print a paper copy of the receipt Download - Copy an electronic copy of the receipt to your computer Pay Again - Return to the payment screen to make another payment Close - Close the payment window.
Choose "Close" if you are finished. Note: If you are using a public computer, we recommend that you return to the payment screen, and press "Clear" to erase your information. However, if you forget to do this, only the last 4 digits of your payment account will show, so no one else have access to your account.
