Tech notes: Match meds to help improve Heart Failure Core Measure compliance

Up to 90 percent of failures to meet the Heart Failure Core Measure result from a medication mismatch between the medications listed in the discharge summary and those listed on the After Visit Summary (AVS).
Here are two ways in Epic to avoid a medication mismatch.
- Use the default discharge summary template — IPMEDICINE DISCHARGE SUMMARY:TXT, 22886 and the “.dcmedlhs” link, which allows you to match the meds that will appear on the AVS.
- Right click on the green links and refresh the med list just prior to signing the document. If a new med arises after you have already signed the documents, you can create an addendum and refresh the med list to reflect any updates.
