Coded and Anonymous Genetics Research

The state of Oregon requires all direct health care providers to ask their patients whether they want to have their medical information and/or biological specimens (blood, urine, or other tissue specimens collected from your body) used for "coded" or "anonymous" genetic research.

This type of research uses medical information or biological specimens gathered as a part of your clinical care. It does not require any extra contact with you. As it is anonymous, the medical information and/or specimens cannot be linked to you.When you are admitted as an inpatient or outpatient for services at Legacy Health System, you will be asked to read and sign the following form.

Notice of Your Right to Decline Participation in Future Anonymous and/or Coded Genetic Research

By marking the box of your choice on this form, you can tell us whether or not you want your health information and/or biological samples to be available for use in research. It's your choice. Your decision will not affect the care you receive from Legacy Health.
