Guidelines

Legacy uses bioaffinity chromatography, which is not affected by heterozygous hemoglobin variants. Conditions such as homozygous Hb S, Hb C, Hb E, and Hb D can result in decreased life span of the red blood cells which will cause HbA1c to be lower than expected, regardless of the method used. If patient has a homozygous variant, consider alternative testing in order to determine glycemic control.

Comments

*A lower goal of <7.0% for 13 - 19 year old patients or <6.5% for >19 year old patients is reasonable if it can be achieved without excessive hypoglycemia or other diabetic complications.

Estimated Average Glucose (EAG):
Estimated average glucose (EAG) is calculated from HbA1c using the equation established in the A1c-Derived Average Glucose (ADAG) Study. An HbA1c greater than or equal to 6.5% is the threshold for diabetes, which corresponds to an EAG result of 140 mg/dL.
