Blood Product

Description

Contains at least 3x1011 platelets in 200-400 mL plasma. Also contains hemostatic levels of coagulation factors. May contain trace to 2 mL RBCs, 0.3 to 6x109 WBC.

Patient Preparation

Preinfusion platelet count usually indicated. Patient should have patient identification wristband for comparison with component container label before administration. See the LHS Blood Administration Policy.

May not be immediately available at all sites.

Reported

Allow a minimum of 1 hour for routine products. One to three days may be required for special requests, such as HLA matched products.

Comment

One platelet pheresis is equivalent to 6-8 random donor platelets but is from one donor. Available as a leukoreduced product containing <5 X 106 WBC. ABO compatibility is desirable, but not required. Rh compatibility desirable in women of potential childbearing age. Crossmatch is not necessary. HLA-matched donors may be needed for refractory patients. 

Use

Bleeding related to thrombocytopenia or platelet dysfunction.

Product is prestorage leukoreduced and may be utilized when a CMV reduced risk product is warranted.

Dosage

1 unit is the usual hemostatic dose in adults, with a usual platelet increment of 20-30,000/unit. Pediatric dose is approximately 10 mL/kg

Administration

Infuse 5 mL/minute or as patient tolerates. Do not warm. Must be administered with an in-line (170-260 micron) filter.
