Guidelines

Currently this test is only available for Legacy Emanuel inpatients.

The test priority will be in the following order:

1. Level 1 – MTP trauma patients
* MTP = Massive Transfusion Protocol

2. Other in-house patients on Level 1 MTP, or other massively bleeding patients (at the request of the anesthesiologist).

3. All other requests for ROTEM will be performed on a first come first serve basis. Physicians must contact the Emanuel Lab (ext. 34664) to ascertain if the lab has the resources to provide the service.

Interpretation is performed by the anesthesiologist.

Specimen Collection Requirements

Collect

Whole Blood: one blue (2.3% Sodium Citrate) top tube (Venous only; arterial blood is not acceptable)

NOTE:  If other coagulation tests are ordered, separate blue top tube(s) must be submitted.

Handling

Mix by gentle inversion, do not vigourously mix. Send tube unopened. Pneumatic Tube System is allowed but not preferred. Handle all orders as STAT.

2.7 mL whole blood

Minimum Volume

1.8 mL whole blood

Rejection Criteria

Clotted samples, samples not maintained at room temperature, collection tubes less than 90% filled

Samples collected and transported from other sites. 
Testing is available for EMANUEL INPATIENTS ONLY!
