Guidelines

Please see Syphilis Testing Algorithms

Order test if the following were performed at a separate laboratory:

1.) Confirming reactive quantitative non-Treponemal screen (i.e., RPR with titer) in the “Traditional” algorithm

or

2.) Confirming non-reactive non-Treponemal tests (i.e. RPR) that were originally screened reactive by immunoassay in the “Reverse” algorithm.

Testing by TPPA assumes that screening was performed at a separate laboratory.  In the “Traditional” algorithm, TP-PA is recommended to confirm reactive non-Treponemal tests (i.e., RPR).  In the “Reverse” algorithm, TP-PA is recommended to confirm non-reactive non-Treponemal tests (i.e., RPR) that were originally screened by immunoassay.

Specimen Collection Requirements

Collect

Serum, one 5.0 mL gold (SST) or 7.0 mL red top tube

Handling

Allow serum to clot completely at room temperature (minimum: SST - 30 minutes and red top tubes - 60 minutes). Separate serum from cells within 1 hour of collection.

0.5 mL Serum

Minimum Volume

0.25 mL Serum (0.6 mL minimum whole blood draw)

Transport

Refrigerated (2-8 ºC)

Rejection Criteria

Hemolyzed specimens, Contaminated with bacteria, Grossly lipemic or turbid specimens, Plasma, CSF

AFTER SEPARATION FROM CELLS: Room Temperature (18-26°C): 24 hours; Refrigerated (2-8°C): 5 days; Frozen (≤ -20°C): 2 months
