Guidelines

This test is appropriate for samples from moist lesions, or lesions on mucous membrane. For CSF specimens, the preferred test is VZV by PCR.

Specimen Collection Requirements

Collect

Use a sterile needle to lift the cap of the vesicle, vigourously swab the base of the lesion to collect the cells. Best if done within the first 3 days of the rash.

Handling

Place swab in viral transport media immediately, or roll the swab across the surface of a clean, dry microscope slide and let air dry. Fix slide with acetone by flooding and air drying before transport.

Transport

Swab in viral transport medium, refrigerated. Slide smear fixed by sender, room temperature.

Rejection Criteria

Body fluids, or swab of mucous membrane with no evident lesion. These samples should be tested using PCR method or culture, though culture is less sensitive.

Comments

Sensitivity of this method is dependent on adequacy of the smear.  If there are fewer than 20 columnar epithelial cells present on the slide, the result will be reported as inadequate.  The antigen will be found within the lesion cells collected, not as free virus in the vesicular fluid.
