PCPs to get new and improved ED visit documentation

Primary care providers will soon be receiving a new and improved Legacy Health “ED to PCP” patient communication document. Implementation is set for late March.

Over the last several months Legacy's Informatics team has been working with our outpatient colleagues to improve the information they receive about their patient's care in Legacy emergency departments.

The new document will be sent either via fax or to their Epic InBasket.

The new version will have essential information that will encapsulate the ED visit in a logical sequence and will include the emergency physician's full note.

Primary care feedback incorporated

Legacy Informatics representatives met with non-Legacy and Legacy employed primary care doctors to get their feedback on how to improve ED-to-PCP communication. A workgroup compiled survey and clinic visit results as well as suggestions from various groups around the system. Legacy Meridian Park Medical Center spearheaded this process.
