New care summary keeps you informed about your patient’s care

Legacy Health has begun meaningful use stage 2 reporting with the start of the new year. Changes to our Epic system associated with the required measures took effect on Wednesday, Dec. 18, 2013.

One change you may have already noticed is the summary of care (SOC) document for all care transitions (referrals or hospital discharges) except emergency department encounters (this report will remain the same). The content of the report is prescribed by the Centers for Medicare & Medicaid Services.

Delivery options

This change means that at specific points of care, your practice will receive referral or discharge reports. These reports are mandatory, but do come with options on how they are delivered to you. Options to receive the SOC are:

Inbasket:

  1. Legacy Epic Care Link or Epic Hyperspace inbasket
  2. Direct message through your electronic health record (EHR) vendor (please contact your vendor to receive messages via your EHR)

Fax:  Default delivery method
