Results and kudos - 2016 Clinical Integration Program results

In a word, the LHP network’s performance in 2016 was OUTSTANDING. 

We met 16 of our 22 network measures, including 4 wellness targets for adult cancer screenings and well-child visits. $1,193,262 in Clinical Integration Program incentive funds were earned by 156 practices, and LHP realized $1.1 million in shared savings through our performance on the Legacy Employee Health Plan contract.

In addition to the results highlighted in the Impact Summary, we achieved these quality outcomes:

Members attributed to an LHP provider compared to a non-LHP provider were more likely to:
• Have a shorter inpatient length of stay 
• Have a well-child visit for all the age bands tracked (0-15 months, 3-6 years, and 13-17 years)
• Be screened for breast cancer
• Receive an eye exam and an HbA1c blood test (diabetic patients)
• Have appropriate asthma and CAD medication utilization 
• Have a drug filled with a generic rather than a name brand drug for tracked chronic conditions
• Be admitted to an in-network hospital

In addition, LHP-attributed members were less likely than non-LHP attributed members to:
• Have an ED visit
• Be readmitted to the hospital following an inpatient stay

Cost savings

The $1.1 million in shared savings realized through our performance on the Legacy Employee Health Plan contract was due in part to:
• More than 20% year-over-year savings for LEHP contract lives managed by Care Support Resources 
• Lower average inpatient length-of-stay (3.3 days) than our Clinical Integration Program goal of 3.5 days
• Meeting our target for generic drug utilization: 96.4% of prescriptions were filled with generic drugs for selected drug classes
• 47 fewer ED visits compared to 2015

Positive cost trends demonstrate LHP’s impact

• Legacy Employee Health Plan costs stayed stable in 2016 after two years of increasing costs
• Legacy Employee Health Plan medical costs fell in 2016 after two years of significant increase
• Legacy Health Partners performance on per-member, per-month expenditure was favorable to target by $9
