Advance your patient care with Legacy Care Support Resources
In the 18 months since Legacy Care Support Resources (CSR) launched, the team has been hard at work, partnering with primary care providers to support over 600 members, helping them forge a path to better health.
The results of the program have been impressive:
* In 2016, current and graduated CSR participants saw a 33 percent reduction in inpatient admissions, a 21 percent reduction in visits to the emergency department and a 31 percent reduction in medical costs.
* To date, the Care Support Resources team has successfully graduated over 300 members. Another 322 are enrolled or in the process of being enrolled.
* Members participating in CSR report a high degree of satisfaction with their experience in the program.
The Care Support Resources team includes nurses, pharmacists and health coaches. The foundation of the program is the partnership between the CSR team and primary care providers. The CSR team works with members to improve their health based on a treatment plan from the provider.
CSR is focused on adults with one or more of the five supported conditions: Asthma, Chronic Obstructive Pulmonary Disease, Congestive Heart Failure, Coronary Artery Disease and Diabetes. Legacy identifies members who would benefit from participating in the program based on claim data, prescriptions, and patient health records. Providers can also refer patients into CSR if they are members of a Legacy Health Partners contracted population.
New service offers on-demand consulting with CSR experts
Care Support Resources will continue to be available to qualified members of LHP contracted populations at no cost to providers. To better support LHP providers, this fall Legacy will begin previewing a new Legacy Partner Solutions offering that gives LHP providers access to experts on the CSR team to consult on any patients whether he or she is a member of an LHP contracted population or not.
With this new CSR consultation line, providers can get on-demand access to a pharmacist, social worker or nurse to consult on patient cases. These Legacy-employed experts are available to collaborate with you in real time to help you address patient needs.
