1. Have you done all you can to ensure you will meet this year’s minimum clinical integration score of 50%? It’s an easy goal to attain if you’ve completed applicable provider/practice measures. Providers: complete a Learning Module by Dec. 29. Practice administrators: make sure you have attested to completion of all practice-level measures.
2. Use your LHP Dashboard to see which of your attributed patients are not meeting applicable measures. Pay particular attention to screenings needed by your diabetic patients – network performance is currently lagging last year’s. Also, be sure to conduct any outstanding well-care visits prior to Dec. 31.
3. Use the manual data submission process for cancer screening measures to improve network performance. You can continue to submit data until February 2, 2018.

2017 network highlights
As we close out the year, we can all feel good about what we collectively achieved and think about how we can apply those experiences and learnings to 2018. Accomplishments include development of a three-year strategic plan, 265 new providers joining the network, great performance on patient safety measures, more contracts resulting in a three-fold increase in covered lives, and new LHP network and practice directories to help you get connected. Facilities can now be included in new insurance products
Expanding the scope of the LHP network is part of LHP’s strategy to improve care quality and reduce costs. There’s now a way for certain types of facilities or practice models to be considered for inclusion in new insurance products that use LHP’s provider network. The list of eligible facility types and more information is included in the Position Statement. Applications will be reviewed by the LHP Quality and Membership Committee and submitted to the LHP Board for approval.
