Where to park:

Free valet parking is available for all appointments at the main entrance of the hospital on NW 22nd Ave, between NW Lovejoy and NW Marshall.  You can also self-park in one of the three parking garages attached to the hospital. 

 

Where to check-in:

Enter at the main hospital entrance located on NW 22nd Ave. Take elevator 1 or 2 to the second floor. As you exit the elevator, the radiology registration desk will be directly in front of you.

For breast related imaging, please check in at the Breast Health Center located in Building 2 on the Legacy Good Samaritan Campus (across the street from the main hospital - 1040 NW 22nd Ave). As you enter the building, take the elevators to the lower level. Exit the elevator to the left and you'll see the receptionist. Self-parking is easiest in parking structure #2 for breast related imaging appointments

 

What to bring:

Please remember to bring your insurance card, photo ID, and any paperwork that your physician provides to the appointment. If you are having an exam that requires a contrast, please bring a list of the medications you are currently taking.
