    Adults, children and their families can experience physical, emotional, social and spiritual comfort from Legacy Hospice. We care for both terminal illness and life interruptions. Learn more about hospice care.

    You may prefer to be cared for in the comfort of your home, with family and friends by your side. Our program is primarily home-based, in the Portland metropolitan area and communities throughout the Willamette Valley. You can rest assured you or your loved one are receiving the best care. All services provided by Legacy Hospice are accredited by The Joint Commission, the nation's predominant standards-setting and accrediting body for health care.

    Legacy Hospice also provides care in residential or assisted living settings, adult foster homes and nursing care facilities. If you are an adult who needs acute symptom management, you may consider our 12-bed inpatient facility in southwest Portland called Legacy Hopewell House Hospice

    A dedicated team

    Our hospice services are provided under the direction of a physician and an interdisciplinary team:

    • Registered nurses
    • Hospice aides
    • Physical, occupational and speech therapists
    • Medical social workers
    • Medical director
    • Spiritual counselor
    • Pharmacists
    • Pediatric consultant
    • Bereavement counselors
    • Volunteers
    • Dietary counselors

     

    With your guidance, the physician and hospice team will design a care plan that may include:

    • Visits by nurses and other health care professionals
    • Management of pain and symptom control
    • Medical treatments as prescribed
    • Palliative care
    • Patient care instruction for family members
    • Personal care
    • Providing or arranging for respite care
    • Assistance in obtaining medical equipment, supplies or medications
    • Counseling and emotional support for patient and family
    • Spiritual support
    • Companionship and practical services by volunteers
    • Information about insurance, financial aid, in-home support, transportation agencies and community agencies
    • Bereavement follow-up for families

     

    Legacy Hospice can also make special arrangements for:

    • Short-term inpatient care for symptom management
    • Medical equipment and supplies
    • Drugs and biological supplies  

    Admission criteria

    Anyone may request hospice services, but a physician's order must be obtained before we provide care. To be eligible for home hospice care, patients must meet these criteria:

    • Terminal medical condition with a life expectancy of six months or less, assuming the illness runs its natural course
    • Agree to palliative care (control of symptoms and not cure of the illness)
    • Agree to follow the plan of care established by the patient, their family, their physician and the hospice team
    • Live within the geographic area served by Legacy Hospice--Portland or Legacy Hospice--McMinnville
    • Have a place of residence (home, foster home or residential care facility) that is a safe environment for the patient, caregiver and hospice staff
    • Have a realistic plan for the provision of primary caregiving 24 hours a day

     

    Legacy Hospice will not knowingly admit or continue to care for anyone whose home environment will not support our services or whose needs cannot be met by our agency either directly or indirectly through service agreements with other providers.
