CALCIUM, IONIZED, WHOLE BLOOD

Mnemonic

CAIW

Synonym

Ionized Calcium WB - Calcium Ionized Level - Calcium Free - Calcium - iCa

Guidelines

To be used for Inpatient, ED and/or surgical testing.

Specimen Collection Requirements

Collect

Whole Blood:  one heparinized blood gas syringe

Arterial or Venous samples acceptable

Handling

Collect specimen anaerobically. Blood gas syringe should be capped with no air space.

All specimens should be collected under anaerobic conditions. For anaerobic samples, do not remove the top of the tube before analysis. Any loss of CO2 will increase the pH, which decreases ionized calcium. Do not store specimens on ice. Whole blood should be analyzed within 30 minutes of collection.

Minimum Volume

0.5 mL

Transport

Room Temperature, unless delayed > 30 minutes.

If testing will be delayed, place specimen on wet ice.

Rejection Criteria

EDTA, citrate, oxalate and sodium fluoride, severely hemolyzed specimens, clotted specimens.  PST or other heparin vacuum tubes are not acceptable for whole blood ionized Ca testing.

Stability

WHOLE BLOOD: Ambient (20-25°C): 30 minutes; Refrigerated (2-8°C): 4 hours; Do not freeze.
