Specimen Collection Requirements
Serum, one 5.0 mL plain red top tube. Also acceptable: Plasma, lavender (K2 or K3EDTA), pink (K2EDTA), green (Heparin) or gray (Potassium Oxalate or Sodium Fluoride).
Separate serum or plasma from cells ASAP or within 2 hours of collection. Transfer 2.0 mL serum or plasma to an ARUP Standard Transport Tube.
CRITICAL FROZEN. Separate specimens must be submitted when multiple tests are ordered.
Whole blood. Gel separator tubes, light blue (citrate), or yellow (SPS or ACD solution).
AFTER SEPARATION FROM CELLS: Ambient: Unacceptable; Refrigerated: Unacceptable; Frozen: 14 days

3. Dose Frequency - Indicate how often the dose is administered (per day, per week, as needed, etc.)
