Services
Our clinical pharmacists care for adults who take complex, critically important prescription medications for:
- Anticoagulation
- High blood pressure
- High cholesterol
- Hepatitis C.
In addition, this service can help patients who take more than 10 medications or have medication management concerns. These services expand upon the Anticoagulation Clinic that have been helping patients with their anticoagulant medication for more than 30 years.

Where to park:
Free valet parking is available for all appointments at the main entrance of the hospital on N.W. 22nd Ave., between N.W. Lovejoy and N.W. Marshall streets. You can also self-park in one of the three parking garages attached to the hospital. Ask for parking validation when you check-in for your appointment.

About
Legacy Medication Management Services require a referral from your doctor, who must have Legacy Medical Staff affiliation. Your doctor should provide:
- A completed Legacy referral form
- Current medication list
- Current medical problem list

Medicare patients, please note: Some of our clinics are considered "hospital-based" because they share the same licensure with a Legacy hospital. The services or procedures you receive at Legacy's hospital-based clinics may include a facility charge in addition to a physician charge.
