Legacy Health Partners - News & Updates

Delivering value and improving health

Two new leaders announced

Last week, Legacy’s president and CEO, Kathryn Correia, announced that Legacy will welcome two new senior vice presidents in July, our chief medical officer and our chief nursing officer. Seth Podolsky, MD, and Kecia Kelly, DNP, are dynamic leaders who come to us with diverse professional and personal experiences that will be essential to Legacy’s strategy and transition to become a health management company. Their past leadership in initiatives focused on value, quality and improved health outcomes are attributes that will both complement and strengthen the work of our LHP network and goals.

Quality Corner: LHP Reporting Suite: Data and tools at your fingertips

The LHP Reporting Suite is your online resource for dashboards, reporting and tools which provide actionable data and evaluate how well we deliver high quality patient-focused care, enhance care coordination and effectively use resources for all patients enrolled in LHP payor arrangements.

The first step to improvement is knowing and managing the information that is already available. The reporting tools available in the LHP Reporting Suite help members in our network track and monitor quality outcomes and view cost and utilization trends to determine the best areas to focus performance improvement and cost/waste reduction efforts.

Learn about specific ways to incorporate these LHP Reporting Suite tools into your clinical practice by selecting your focus below:

CMS to require Clinical Decision Support documentation with advanced imaging orders

The Centers for Medicare and Medicaid Services (CMS) will soon require consult of appropriate use criteria (AUC) through a qualified clinical decision support mechanism (CDSM) for advanced imaging orders. Starting this summer, Legacy Health will request documentation of an AUC score from a qualified CDSM with each advanced imaging order placed for Medicare-insured patients. This includes orders for CT, MRI, PET-CT and Nuclear Medicine. Advanced imaging orders received by Legacy that do not meet new documentation requirements will still be accepted, however the ordering practice will receive a fax to ask for an AUC score.

Announcements, deadlines and reminders

Reminder: Care Support Resources add an extra layer of support at no cost to you and your patients
Legacy Care Support Resources (CSR) offer an added layer of support at no cost to both providers and their patients who have specific conditions. The CSR program expands your patient’s care team to include a care manager, pharmacist, and health coach to help manage conditions and medications, adopt healthier lifestyles and access social services if needed. CSR professionals closely coordinate with providers to manage patient experience, care and cost.

Please review the current CSR Brochure to learn more about the available programs for complex and condition care, eligibility, and opt-in/opt-out opportunities.

Reminder: Complete CI Ed by June 30 for full credit
Please log into E+ to complete your Clinical Integration Education before June 30 to earn full credit. A refresher on how to log in and access the curriculum is available on our Team Site. Keep in mind that LHP providers are required to complete CI Education to be eligible for incentive payouts.

Clinical Integration Education is intended for both providers and clinic administrators and can be completed in 25 minutes. This year’s education reviews LHP’s role in the local healthcare market and acknowledges all we have accomplished as a clinically integrated network.
