Community Health Needs Assessments (CHNAs) and Community Health Improvement Plans (CHIPs)

Legacy Health works at creating a legacy of health for our community and our world-as a locally owned, nonprofit health system, this is our reason for existing; it is why we are here.  From leading efforts to meet the region’s health needs to supporting other organizations to being involved in countless ways, we are crucial for health of the community.

Every three years, Legacy Health assesses the health needs of the community through a Community Health Needs Assessment (CHNA).  We address identified health needs by working in partnership with our community to create a Community Health Improvement Plans (CHIP). Our CHIPs shape our priorities and funding investments. We are committed to addressing the social determinants of health through our community benefit efforts. Addressing the needs of the community is a huge part of our overall community benefit efforts. It is also a measurable approach to improving community health.
