Dear Legacy Health Patient,
I am writing to share important information about your health insurance coverage. After careful review of our contract with Humana Medicare Advantage, Legacy Health has made the difficult decision to end our agreement effective December 31, 2026.
Legacy regularly reviews insurance contracts to ensure they accurately reflect the rising cost of healthcare. After reviewing our agreement with Humana Medicare Advantage, we found the difference between reimbursement rates and the actual cost of care is not financially sustainable.
We recognize the stress this situation creates for you. We are grateful to be your healthcare provider and hope to continue serving you. We are sharing this information now to help make this a smooth transition. If you would like to continue to access Legacy providers and services beyond December 31, 2026, you will need to enroll in Original Medicare or choose a different Medicare Advantage Plan.
Medicare Annual Open Enrollment runs from Oct. 15 to Dec. 7, 2026. This is the time to review your options and make changes to your coverage for the upcoming year. There are many Medicare Advantage plans that include Legacy Health as part of their network. You can find a list of plans we contract with at legacyhealth.org/insurance.

We sent this letter to Legacy Health patients living in Oregon and Washington who we believe are members of the Humana Medicare Advantage plan. If you are no longer insured by Humana, you may disregard this letter.
How can I continue to see my Legacy doctor?
To remain in-network with Legacy and continue to see your doctor, you need to enroll in Original Medicare or choose a different Medicare Advantage plan. Medicare Annual Open Enrollment is Oct. 15 to Dec. 7, 2026. This is the time to make changes to your coverage for the upcoming year.
