We are ready to answer your questions and help you schedule a future in-person primary care appointment. Join the Legacy Better Health program
The Better Health program is a free, personalized service that supports you and your health goals. When you sign up, you’ll have access to a care management team which may include nurse care managers, clinical pharmacists, and health coaches. Your care team can help you manage or improve an ongoing condition, make healthy lifestyle changes and more. Call us to learn more and register.

We offer virtual appointments and in-person visits at many locations throughout Oregon and Southwest Washington. With six hospitals, many urgent care clinics, and primary care and specialty providers, we are dedicated to your total health and wellness – not just treating you when you’re sick.
Should you go to the emergency room or urgent care clinic?
