What is happening between Legacy Health and Cigna?
Legacy Health and Cigna are negotiating a new contract for commercial plans that will determine how much Cigna pays Legacy for the care we provide its members. Legacy has requested a fair, reasonable increase to meet rising healthcare expenses. Legacy is in active negotiations with Cigna. We hope to reach a resolution soon.
How does this affect me as a Cigna member and Legacy patient right now?
- There are no changes to your insurance coverage right now.
- Legacy Health wants to reach an agreement with Cigna before the current contract expires at 11:59 p.m. on June 30, 2025.
- We have had a positive, productive and long-standing relationship with Cigna. While we have not yet reached a new contract with Cigna, both of us are still working collaboratively and intend to continue negotiating. We are hopeful they will agree to a fair and reasonable increase in payment before June 30, 2025.
- If we reach an agreement, there will be no disruption to your care at Legacy as a Cigna member.
Is the entire Legacy Health system of hospitals or clinics affected by the Cigna contract?
No. The negotiated contract does not include the Legacy hospital or nearby clinics in Silverton. They are under a separate agreement with Cigna. For clarity, here is a list of facilities that will not be affected:
- Legacy Silverton Medical Center
- Legacy Medical Group–Behavioral Health in Keizer
- Legacy Medical Group−Molalla Family Medicine
- Legacy Medical Group−Mt Angel
- Legacy Medical Group–Orthopedics and Sports Medicine in Keizer
- Legacy Medical Group−Silverton
- Legacy Medical Group−Silverton Family Medicine
- Legacy Medical Group–Women’s Health in Keizer
- Legacy Medical Group−Woodburn Health Center
I have a Cigna Medicare Advantage plan. Am I affected by the contract?
- No. Nothing changes. You can continue to see Legacy Health providers and access our services as usual.
Why did Legacy Health ask for a rate increase?
- Cigna payments to Legacy have not been adjusted to meet rising inflation costs or to balance record high expenses.
- We have asked Cigna to pay its fair share of the more than 35% increase in costs since 2022 to operate our healthcare system.
- You may have heard that we issued a notice of termination to Cigna. This is a standard step during contract negotiation that merely prevents the current contract from automatically renewing at existing rates.
What happens if an agreement can’t be reached?
- If an agreement is not reached, Legacy Health hospitals, clinics and urgent care centers, including Legacy–GoHealth Urgent Care centers, will no longer be in-network for Cigna members after June 30, 2025. This means Legacy will be out-of-network for Cigna members.
- Out-of-network means that you may have to pay more money out of pocket for services at Legacy.
I have an appointment scheduled after June 30, 2025. What does this mean for me?
- Legacy staff are actively contacting impacted Cigna patients who have health care appointments scheduled in July.
- Patients have several options to choose from, including:
- Cancelling their appointments
- Paying out-of-network rates (which will likely result in higher out-of-pocket costs)
- Exploring financial assistance programs with Legacy
- In some cases, patients may still be able to keep their appointments at in-network rates for a limited duration, pending approval from Cigna. This is known as Continuity of Care. If you have questions, please call Cigna directly at the phone number on the back of your member ID card. You can also learn more and access Cigna’s continuity of care application form here.
- Please note: We are still scheduling new appointments with impacted Cigna patients after June 30. However, these appointments may be cancelled if Legacy and Cigna do not reach an agreement.
I will be in ongoing treatment past June 30, 2025. Will Legacy continue to be covered as in-network by Cigna insurance?
- Some patients may still be covered as in-network for a period of time, if they qualify for continuity of care.
- Continuity of care allows patients who qualify to continue seeing their provider at in-network cost for a limited time.
- Unfortunately, Legacy does not have the authority to make decisions about your insurance coverage or determine if you qualify for continuity of care.
- If you are being treated for any condition that requires ongoing care and you have questions about your insurance coverage, please contact Cigna directly. Your insurance company is responsible for making decisions about your insurance coverage if the contract ends. You can call the phone number on your member ID card.

What are my options if Legacy Health becomes out-of-network for Cigna members?
- Check your plan benefits to see if you have out-of-network coverage. Some Cigna plans may cover some or all out-of-network care costs, depending on the deductible, coinsurance and out-of-pocket maximum. Call the phone number on the back of your member ID insurance card to learn more.
- Explore other health insurance options that include Legacy Health as an in-network provider. You may be able to switch to a different insurance plan during open enrollment. Also, you may switch plans if you experience a qualifying life event, such as moving, getting married or losing your job. Visit oregonhealthcare.gov to compare plans and see what works best for you or talk to your employer’s benefit department.
- Apply for financial assistance from Legacy. We have financial assistance programs for patients who qualify based on income and family size. To learn more, visit LegacyHealth.org/FinancialAssistance.

What can I do to help?
- Contact Cigna using the phone number on the back of your member ID card and ask them to support Legacy Health’s request for a fair and reasonable payment increase.
- Tell Cigna how important Legacy is to you and your family and urge them to reach an agreement before June 30, 2025.
- Share your feedback and concerns with your employer. Employers often have influence over health plan choices.
