How do I make a referral for a Developmental Pediatric Evaluation?
At the date of the referral, child must be 12 months to 4 years 11 months of age.
- The doctor is asked to fax patient documentation: pertinent medical records, demographic information, and specific questions to be answered. Please include clinical presentation.
- Follow-up with Pediatric Development RN Care Coordinators at 503-413-2600 for additional questions.
