Quality Corner: Strategies for improving Timeliness of Discharge Summary measure
As a Network, LHP providers have not met the performance target on the Timeliness of Discharge Summary measure in the past two years. Current performance of 83.7% for 2018 is also falling short of the 85% year-end target. With direction from LHP’s provider-led Clinical Collaboration and Performance Improvement Committee, we’re taking steps to improve network performance by focusing on certain specialties. September 30 deadline reminders
• Submit patient re-attribution requests for 2018. Accurately attributing patients aids in closing care gaps, improving performance on LHP’s risk-sharing contracts, and impacts providers scorecards and incentive payments. Get the information and forms you need on the LHP team site.
• Complete the 2018 Clinical Integration Education. It’s an easy way to help earn points toward this year’s minimum 60% clinical integration score and be eligible for an incentive payout.
• Attest to two membership requirements: Medical Home for primary care practices and Certified EHR Technology (CEHRT) for all practices.
Honing in on Headaches: Oct. 4 Clinical Collaboration Series event
Mark your calendar for October 4, 6-8:30 p.m. when LHP Network neurologists will explain common types of headaches seen in primary care, headache management, updates in treatment and appropriate imaging. Outreach to unattributed members about preventive screenings
Approximately 2,500 members of LHP’s contracted populations will soon be receiving a postcard reminding them that they are behind on one or more preventive screening measures.
