Reminder: Address care gaps before the end of the year
The 2019 LHP Clinical Integration (CI) program dashboard in the LHP Reporting Suite is a great tool for viewing current performance and identifying patients who have outstanding gaps in care. Performance for the following chronic disease measures were posted recently and there is opportunity to improve the medication dispense and adherence measures before the end of the year, so be sure to check your lists on the "Opportunities" tab and help address any care gaps for your patients.
- Diabetes care: Medical attention for nephropathy
- Statin therapy for patients with diabetes
- Appropriate testing for children with pharyngitis
- Medication management for people with asthma
- Statin therapy for patients with cardiovascular disease
- Persistence of beta blocker therapy
Preview of the 2020 Clinical Integration program
The Quality and Membership Committee recommended – and the LHP Board recently approved – the 2020 clinical integration (CI) program. Get help: Brief Epic update online training available
The next quarterly Epic update is scheduled for Wednesday, Oct. 16. The update includes improvements to make Epic easier to use and is now scheduled quarterly to minimize the amount of change to providers. The best way to prepare is to complete the 15-minute udate training. Please note: If accessing the training materials from outside the Legacy network, first login to the Legacy Portal for providers and then select Epic Quarterly Update Training from the “top links” menu. Legacy Medication Management Services now accepting referrals
Providers can now refer patients into Legacy Medication Management Services, a new service that is available to help patients with complex, critically important prescription medication regimens. Expanding on the anticoagulation clinics Legacy has operated for more than 30 years, this new service offers support for patients this new service offers support for patients managing medications for hepatitis C, hypertension and/or hyperlipidemia, as well as polypharmacy patients. New health plans from PacificSource Health Plans coming in 2020
Beginning in 2020, there will be a greater variety of health plans from PacificSource in the market and the plans will have new names. One of the key distinctions between the different plans is the level of network flexibility that they offer for referrals, so it will be important to understand the differences between each plan.
