Get help with your Patient-Centered Medical Home certification

Certification as a Patient-Centered Medical Home is a membership requirement for primary care practices in the Legacy Health Partners network. In 2018, LHP recognized practices that were in the process of obtaining Medical Home recognition as meeting the membership requirement. To maintain membership, primary care practices are expected to be fully certified by December 31, 2019.

If your practice is not yet certified as a medical home, LHP may have some resources to help with it. Legacy Partner Solutions for Patient-Centered Medical Home is a service offering to help LHP members with medical home certification. Grants to pay for this service may be available to smaller practices through Legacy Health Partners.

Legacy Partner Solutions for Patient-Centered Medical Home offer flexible support for primary care practices working towards medical home recognition through either the Oregon state PCPCH program or the NCQA program. Services include support for things like conducting a medical home readiness assessment, guidance on the application, training on the processes necessary to meet certification requirements, and consulting around opportunities for panel coordination.

We have heard from several smaller practices that affording the resources necessary for medical home certification is a challenge. That is why Legacy Health Partners has made a limited amount of grant money available to smaller LHP practices (1-10 clinicians) who are in good standing with the network to use towards these medical home services. We think that having all primary care practices achieve medical home certification will elevate the performance of this network, so it is the right investment to make to help smaller practices get there.

The grants will be approved through the Population Health Services Committee and are available on a first come, first serve basis. Interested practices should contact their field operations adviser for more information.

Legacy Partner Solutions for Patient-Center Medical Home is part of the Legacy Partner Solutions suite of clinical and administrative support services designed to help LHP members meet their population health goals. In addition to medical home services, current offerings include services for MACRA, patient satisfaction surveying, and a Care Resources Line to help with pharmacy and resourcing inquiries.
