Apply now for an LHP Quality Award
Legacy Health Partners is now accepting applications for the LHP Quality Awards. These awards are open to both primary care and specialty groups that apply by Oct. 15. The purpose of these awards is to recognize important performance improvement projects taking place across the LHP network, celebrate results in improvements in care/processes/service, share quality principles and learnings across the network, and encourage widespread adoption of quality improvement efforts. Minimum clinical integration score is 60 percent for 2019
The Legacy Health Partners Board has approved a clinical integration (CI) score threshold of 60 percent that LHP members must meet to be eligible for any CI incentive for the 2019 performance year. The minimum threshold supports network goals for provider engagement and high performance. Any LHP provider who does not meet the minimum threshold will not earn a CI incentive for 2019 and will be placed on a monitoring plan for 2020. September 30 deadline reminders
Complete 2019 Clinical Integration (CI) Education in E+ by Monday, Sept. 30. It’s an easy way to help earn points toward this year’s 60 percent minimum clinical integration score and be eligible for an incentive payout. If you need help, instructions are posted on the LHP team site.
Submit any patient reattribution requests for 2019. Accurately attributing patients aids in closing care gaps, improving performance on LHP’s risk-sharing contracts, and impacts providers’ scorecards and incentive payments. If you think a patient has been incorrectly attributed to you and you know whom the patient should be attributed to, you can submit a reattribution request by Sept. 30 to have the patient reattributed.

New method to access the LHP Team Site
Legacy Health has launched a new portal called MyPortal (MyPortal.lhs.org), which has replaced LHRemote and is your primary access portal to many Legacy applications and programs. The LHP Team Site is also now accessible using MyPortal. Read the full story for new directions on accessing the LHP Team Site from outside of the Legacy network.
