Challenges from extraordinary year reveal strength
As the last of autumn leaves settle and the winter chill sets in, I wonder how we are this close to welcoming a new year, and I am equally astounded by how much we have overcome to make it this far. In many ways, the past eight months have felt like a single season that brought unimaginable events of extraordinary consequence one after the other, many that continue to challenge us today. With little time to regroup and recharge, it's understandable that each of us feels exhausted and uncertain about the future, but still, the work continues.
Your dedication to your patients' safety and care, and your ability to adapt to their ever-changing needs, demonstrate the true strength and value of our network. While LHP leaders cannot predict the course that the coming season will chart, we can confidently say that we are committed to keeping LHP whole. We will continue to define support for our members to get through whatever challenges our network has yet to face.
Quality Corner: 2021 CI program at a glance
2020 brought many challenges to our network, but our CI program and LHP practices continue to thrive. We are grateful for the unwavering care your practice and providers have administered during this unprecedented time. The Quality and Membership Committee recommended – and the LHP Board approved – the 2021 clinical integration (CI) program, which includes no new quality measures.

Consider Care Support Resources for your highest-need patients

Legacy Care Support Resources offer an extra layer of support to providers in managing the patients among their panel who require the most care and may benefit from services that are unavailable in many traditional clinic settings. LHP providers can refer patients into the program if the patient meets basic requirements.
Patients who enter LHP’s CSR program receive support to help manage their conditions and medications, transition smoothly between care settings, adopt healthier lifestyles, and access social services, if needed. The CSR care team includes case managers, pharmacists and health coaches who work with providers to care for patients in a coordinated, cost-effective manner.

Save the date: Diversity, equity and inclusion provider event
Join us for a virtual educational event focused on the unique opportunity that providers have in creating a culture of equity, safety and respect as well as strategies for better patient outcomes. Nationally acclaimed speaker and health care race and gender equity advocate, Dr. Esther Choo, is our featured speaker for this special event.

Half-credit for CI Education ends in December
