Legacy Health service changes

Like many healthcare organizations, Legacy Health is facing significant financial pressures that require difficult but necessary choices to ensure we can continue delivering high-quality care and fulfilling our mission for years to come. Rising labor and supply costs, stagnant reimbursement rates and broader uncertainties in healthcare continue to challenge our sustainability. Legacy is particularly affected because roughly two-thirds of our patients are covered by Medicare or Medicaid, where reimbursement does not cover the cost of care.

In Oregon and Washington, recent state-level mandates, including mandated nurse-to-patient ratios and expanded presumptive eligibility for financial assistance, have added to these challenges by increasing costs and reducing revenue. Despite ongoing operational improvements and more than 200 position reductions in the past three years, Legacy projects a loss of approximately $38 million this fiscal year. And this is before we experience any negative impacts from HR1, which is expected to bring deep cuts to Medicaid and other safety net programs in the near future.

After a detailed review of services, programs and clinics with the greatest financial challenges, Legacy Health will move forward with several difficult but necessary closures and changes. These decisions were not made lightly and are essential to remain financially viable and ensure Legacy remains here to serve our patients and communities for years to come.

Legacy Health service closures and changes

  • Legacy Devers Eye Institute at Emanuel’s last day of operation will be Jan. 9, 2026.
  • The Emanuel Outpatient Neuro-Rehabilitation program will no longer be available at Emanuel at the beginning of 2026.
  • The Legacy Salmon Creek Pain Clinic’s last day of operation will be March 20, 2026.
  • Good Samaritan Cardiac and Pulmonary Rehabilitation Program’s last day of operation will be Jan. 9, 2026.
  • Emanuel Outpatient Rehabilitation services will narrow its scope to focus on LMG and Trauma patient referrals, beginning in 2026.

Legacy-GoHealth Urgent Care closures — effective Nov. 15, 2025

  • All Washington clinics (Camas, Cascade Park, Hazel Dell and Salmon Creek) will close.
  • Two Oregon clinics (Pearl District and Williams) will close.
  • Eight Legacy-GoHealth Urgent Care centers in Oregon will remain fully open.
LHP providers should be aware of these service closures and updates so they can direct referrals to the appropriate location.
  • Referrals to the Cardiac/Pulmonary Rehabilitation program should be sent to Emanuel, not Good Samaritan.
  • Referrals to the Outpatient Neuro-Rehabilition program should be sent to Good Samaritan, not Emanuel.
  • Referrals to the Legacy Devers Eye Institute should be sent to Good Samaritan or Tualatin locations, not Emanuel.
  • More information will be shared in early 2026 about referrals to the Legacy Salmon Creek Pain Clinic.
  • Urgent care patients can still seek services at one of the eight Legacy-GoHealth locations in Oregon that will remain fully open.
  • Virtual urgent care appointments with Legacy-GoHealth will continue to be available to patients in WA and OR.
  • Other urgent care providers are available in the greater Vancouver and downtown Portland areas.
