This year marks a milestone, 10 years of Legacy Health Partners. A decade of partnership, progress, and a shared belief that we achieve more for patients when we work together than when we stand alone. As we celebrate this anniversary, we want to lift up your voices, because you are the reason LHP exists and continues to thrive.

We’ve been especially inspired by the number of physicians and administrators stepping forward to serve in LHP governance roles. Your willingness to lead and share your perspectives speaks volumes about what this network means:

We also hear how LHP strengthens real-life clinical practice. Dr. Emoniel Isakharov, an Internal Medicine Physician at Legacy Medical Group - Tualatin, described how attending the Clinical Collaboration Series helps him stay current with trending topics where outpatient providers in our region often need support—like mental health, ketamine therapy, and menopause care. “The content shared in these lectures is not only applicable, but very actionable,” said Dr. Isakharov. “Now I can speak to my patients with confidence when they ask me about these topics.”

Equally significant is the sense of community fostered by these events. As Dr. Isakharov put it, engaging with both new and established colleagues face-to-face helps expand that “inner circle” of trusted specialists, turning a large network into a community with more personal connections.

Quality Corner: Announcing LHP’s 2025 Quality Award winners!

LHP is pleased to congratulate Metropolitan Pediatrics as the winner of the 2025 LHP Quality Awards and $2,000 prize for their improvement project focused on increasing HIV screening rates in adolescents via rapid point of care testing to support early detection, linkage to care, and prevention.

Legacy Health’s high value care team earned 2nd place and $1,500 for implementing system-level interventions to reduce inappropriate use of Respiratory Quad PCR testing outside of respiratory virus season.

Legacy Medical Group - Westside Internal Medicine earned 3rd place and $1,000 for improving PCP coverage and patient access through rapid communication with patients and clear transition plans during physician departures/retirements.

As one member of LHP’s provider-led Quality & Membership Committee mentioned, “The quality awards are a valuable reflection of the clinical work we do. I appreciate that LHP makes it a priority to recognize innovation in clinical practice. I intend to take elements of the applications and try to implement in my practice.”

Announcements, deadlines, and reminders

  • LHP offers two learning activities that are due on December 19. LHP has been sending email reminders directly to providers so you know if you have not yet completed these activities yet. We strongly encourage you to do so as soon as possible to ensure you meet the deadline. If you need assistance or have any questions, do not hesitate to reach out to the LHP team for support.

  • The recording of our Clinical Collaboration Series event for Psychedelics and Dissociatives in Chronic Pain Care, held on October 27, is available here. Remember, all recordings for events held this year are listed in LHP’s Resource Library.

  • We are pleased to announce the 2026 clinical integration program to the network. You can view a summary of all 2026 performance measures, membership requirements, and access standards on the LHP Team Site. Early in the year, we’ll continue to share details about the program and how you can best contribute to LHP’s performance and our collective success as a network.
