As we look toward 2026, we do so with optimism and with a clear-eyed understanding of the national landscape shaping value-based care. One thing is certain: value-based care isn’t going anywhere. In fact, it is becoming the norm. Recent CMS results show record performance from Medicare Shared Savings Program (MSSP) Accountable Care Organizations (ACOs), with primary care and physician-led organizations consistently outperforming others on savings and quality. At the same time, federal policy is signaling a continued push toward prevention, patient engagement, lower administrative burden, and greater accountability through shared and downside risk.

Taken together, these trends reinforce what we know to be true at Legacy Health Partners: strong primary care, reliable clinic workflows, well-managed transitions of care, and meaningful use of data are essential, not only for performance, but for sustainability in an increasingly tight reimbursement environment.

We are closely monitoring these developments and intentionally positioning LHP to meet this moment. A major focus in 2026 will be the development of LHP’s new strategic plan. With leadership from our Board and input from across the network, we will chart a path forward that reflects today’s realities while staying grounded in our mission to deliver value and improve health. You will hear much more as this work takes shape.
We also look forward to welcoming new Board and committee members, whose perspectives will strengthen our clinical integration, contracting strategy, membership standards, incentive program and network improvement efforts. Recognizing and learning from our LHP Quality Award winners will also remain a priority, as we share practical, clinic-tested ideas across the network to help our member practices think about how to introduce new concepts and workflows into their own clinics.

Most importantly, we look forward to continuing to hear directly from you. Your insights help shape how LHP supports your practices and advances our shared commitment to better care for the patients we serve, together, and well into the future.

Quality Corner: HPV self-swab testing & cervical cancer screening update

January is Cervical Cancer Awareness Month and is the perfect time to reinforce the importance of routine Pap smears in preventing cervical cancer. LHP’s cervical cancer screening toolkit is available to help providers succeed in screening for cervical cancer and improve patient’s health and experience. LHP launches regional meetings for managers and administrators

Last October was the first LHP quarterly update meeting for managers and administrators. In-person and online attendees both from primary care and specialty care were given timely network updates, education around Hierarchical Condition Category (HCC) coding and review and status of our network quality measures.

The goal of quarterly update meetings is to offer managers and administrators an avenue to learn, ask questions, and network with peers within LHP. The strength of our clinically integrated network lies in the cooperation between our primary care and specialty providers. Building relationships between clinics will foster improved communication and referral workflows.

Be on the lookout for our next quarterly session to be announced for late February/early March. For these meetings to be most useful, we encourage managers and administrators to suggest topic ideas to their Quality Improvement Consultant or Physician Outreach and Engagement Advisor.




Continuing our commitment to Legacy Health Partners in 2026

We value the strong relationships we have built with you and remain committed to supporting your success. As we move into 2026, we want to share updates about how our Provider Outreach and Engagement Advisor team will collaborate with you.

Our team streamlined and each outreach advisor will be covering more clinics and medical centers due to recent organizational changes at Legacy Health. While this means some adjustments in how we connect, our goal remains the same: to ensure you feel supported and informed.

Here is what you can expect:
  • New provider outreach advisors for some clinics: A few clinics will see a change in their assigned advisor. If your clinic has a new advisor your new advisor will reach out to you soon if they have not already.
  • Modified visit frequency: Some clinics may receive fewer individual meeting requests with their advisor, but we will continue to provide timely updates and support via email or phone. We are always available to you by email.
  • Dedicated member coordinator communication: For certain updates, you may hear directly from our LHP Member Relations Coordinator, Kristin Gilmer, to keep you informed.
  • You will be hearing from your outreach advisor in the new year to discuss any changes that will affect you.

2026 Contracting Overview

Legacy Health Partners begins 2026 with ten returning value-based contracts and managing 100,000+ lives. All the details about our contracts, including shared savings and care coordination programs can be found on their one-pagers located on the contracting page (login required) on the LHP team site.

PacificSource Plan update: PacificSource members with individual and family plans will be transitioned from the Navigator product to the CORE product. This change reduces out-of-network access for those members, so LHP providers should see a greater share of the business for our PacificSource commercial business. The Navigator rates for your practice will still apply to the CORE members. Learn more here: Pacific Source CORE Plan Information




Announcements, deadlines, and reminders

  • Adult primary care practices: LHP will be reaching out soon to request supplemental clinical data to maximize your performance for certain 2025 measures that are not captured through claims data but are recorded in a patient’s electronic health record (EHR). The deadline to submit data securely to LegacyHealthPartners@lhs.org will be Feb. 27. If you have questions about completing this process, please talk with your outreach advisor.

  • Practice administrators: Review the attestation report in Power BI (login required) to verify that completions for 2025 practice and provider measures/activities are accurate. If you find anything missing or captured incorrectly, submit an appeal request form by March 6.

  • This month’s Legacy Health Equity Series event covered Digital equity and the current state of telehealth at Legacy Health presented by LHP’s very own medical director, Dr. Albert Chaffin. CME credit is available if claimed by Feb. 8, 2026 (within 30 days of the session date) and the activity could count toward Cultural Competency Continuing Education (CCCE) required by the Oregon Medical Board.

  • Metropolitan Pediatrics was selected as the winner of the 2025 LHP Quality Awards. Read the story from December for a summary of last year’s quality projects and how we are recognizing our finalists.

  • View the 2026 clinical integration handbook on the LHP team site for information about this year’s performance measures, requirements, and expectations to succeed as a clinically integrated network. LHP plans to monitor a few new measures at the network level (Kidney health evaluation for diabetes, Follow-up after ED visits for people with multiple high-risk chronic conditions, Osteoporosis management) to support Medicare Star performance. Performance dashboards for 2026 will be published later this spring as usual to help track performance and contribute to high quality care.
