Legacy outpatient infusion transitions to OHSU management

As part of the OHSU Knight-Legacy Health Cancer Collaborative, OHSU is now managing the outpatient infusion clinics at Legacy Good Samaritan, Legacy Meridian Park and Legacy Mount Hood medical centers. This change applies to all outpatient infusion, not just oncology.

  • Outpatient infusion services phone and fax numbers remain the same.
  • OHSU Managed Care Coordinators will conduct insurance pre-authorization for physicians who request it.
  • Practices conducting their own pre-authorizations should use these numbers, which are the same for all three infusion locations: Hospital TIN: 93-1176109; NPI: 1609824010
  • Physicians using Legacy Epic for ordering outpatient infusion at the Good Samaritan, Meridian Park and Mount Hood locations saw a change. Details were communicated in late August.
  • Physicians referring to these three locations and not using Legacy Epic have a new resource for pre-set order forms: www.ohsuknight.com/infusionorders
  • As of Sept. 3, outpatient infusion services at Legacy Meridian Park and Legacy Mount Hood moved to campus medical office buildings:

    New Meridian Park location
    Medical Plaza Building 2
    OHSU Knight Community Hematology and Oncology, Suite 140

    New Mount Hood location
    Medical Office Building 3
    OHSU Knight Community Hematology and Oncology, Suite 140

In August Legacy began communicating these changes directly to patients via personal letter and reminder phone calls prior to their next appointment.

Many of Legacy’s infusion nurses have stayed with the program, and the collaborative continues to provide the full range of infusion services, including chemotherapy, blood products and hydration on an outpatient basis.
