2018 Year in Review

As we close out the year, we can all feel good about what we collectively achieved. Here are just of a few of our 2018 highlights:
•  Bringing together providers to improve care: LHP hosted two very successful clinical collaboration events in May and October that brought primary care providers and specialists together to discuss appropriate imaging for joints and headaches. We encourage you to attend future events.
•  Increasing the meaningfulness of our Clinical Integration Program: This year we reported select primary care measures at the individual level to help PCPs monitor their personal performance on their LHP Dashboard. New specialty-specific measures like Perioperative Temperature Management and Controlling High Blood Pressure were added to help round out the clinical integration program with specialists in mind.
•  Elevating provider awareness and engagement: More than 93% of full-member providers completed the annual Clinical Integration (CI) Education by the September 30 deadline. We kept members informed through 12 monthly newsletters and 13 special Hot Topics. The LHP Team Site was redesigned to be more user friendly. And your Field Operations Team provided consistent and timely support – in person, on the phone and online – to your practices.
•  Helping practices navigate complexities: The LHP team provided support to three practices for MACRA and MIPS reporting and completed six Medical Home assessments to help primary care practices evaluate their readiness to meet Medical Home recognition requirements.
None of this could have been accomplished without the dedicated work from all of you, the LHP Board of Managers, the board committees, and the outstanding leadership provided by our administrative team.

Growing pains

The end of the year is also a time to reflect on areas where Legacy Health Partners may have fallen short and to identify ways we can continue to improve. LHP is on the cutting edge of data integration and population health technology with Epic and, as with many innovative ventures, we have experienced some growing pains. In recent months, some data challenges impacted our ability to share timely and up-to-date data on LHP dashboards and to provide care gap and attribution reports. We understand the frustration and inconvenience some of these data challenges have caused for practices. We remain committed to sharing data that is accurate while continually optimizing processes to give you access to data that is meaningful and timely. 

New services and resources launched

Despite some of the challenges we tackled this year, we also reached some incredible milestones and implemented exciting new services to further support LHP practices:
•  Legacy Partner Solutions, a portfolio of services and programs to help you and your practice improve patient health, continued to expand in several exciting areas:
o  The Care Resources Line launched this past summer to provide on-demand, telephonic access to consultation with a pharmacist or care coordination specialist for any of your patients.
o  Patient experience surveys are available to LHP practices at a favorable rate through an arrangement with Press Ganey. The service includes Press Ganey surveys, technical support and access to best practices and other educational material.
o  We continue to provide services to support practices with MACRA and MIPS reporting. We evaluated the final rule for 2019 and shared our take on the key changes in the Nov. 29 Hot Topic.
•  LHP also began a library of clinical collaboration guides for co-management between primary care and specialty to reduce unnecessary clinical variation and promote responsible utilization of health care resources. See the November 2018 newsletter for more details.
