This month your LHP Field Operations Adviser will be meeting with LHP practices to review the results of the 2017 Clinical Integration program. Here are a few highlights to whet your appetite.
In 2017, the LHP Network, comprised of nearly 2,500 members representing 190 practices:
• Outperformed targets for 18 of the 24 network measures
• Exceeded 5 of 6 wellness targets for adult cancer screenings and well-child visits
• Earned $1.2 million in incentive funds
• Assumed responsibility for 66,000 lives through LHP contracts, including more than 500 lives managed by Care Support Resources (CSR)
• Reduced inpatient admissions, ED visits and medical costs in the CSR population
The 2017 Impact Summary provides an inspiring overview of your accomplishments. There’s still room for improvement, but let’s take a minute to acknowledge and celebrate our individual and collective achievements. Bravo!.
June 30 deadline to earn full points for CI Education completion

For the past five years the LHP Board, committees and leadership team have focused on building the functional components of our clinically integrated network. Our organization is well positioned to meet the unique and varied needs of area providers, patients and payors.
We’ll continue to build and support LHP’s foundation and operational structure, but we’ve also entered a new stage in our evolution that warrants some introspection. At its annual retreat in May, the board reviewed LHP’s strategic goals in light of the network’s culture and the values that represent that culture. The LHP Board took stock of how well we are living up to our values – partnership, improvement, performance and purposeful. Read the whole story.
Orthopedists share insights about imaging
LHP’s first Clinical Collaboration Series event on May 24 was a great success by all accounts. LHP medical director Nick Kashey, M.D., summed it up this way: “I think the 30 attendees really appreciated the information and were happy to hear concrete ways to bring value to their clinical care. And the presenters enjoyed connecting with their referral base and sharing knowledge that will help the right patients get seen in the right place at the right time.” Read the whole story.
Reports available on LHP Dashboards in Epic
Attribution reports and many of our patient care gap lists are now available on your LHP Dashboard in Epic for primary care providers and certain specialists – such as Obstetrics or Endocrinology – who are eligible for having attributed patients. These reports can be found in the “LHP Reports” section on the right-hand side of your LHP Dashboard or within the Report Library in Epic. Who’s who in LHP – Updated Directories
The LHP directories posted on the LHP team site have been reviewed for accuracy and updated. Use them for patient referrals, to connect with peers, and to get to know who is in the LHP Network. Built as an Excel file, the Network Directory allows you to easily sort and identify providers, their specialty and practice name. Here are instructions to learn how to sort the information by practice name or specialty. The Quick-Look Practice Directory is a PDF that lists all LHP practices alphabetically and their locations for easy reference.
