New Care Resources Line offers on-demand telephonic access to pharmacy and resourcing expert
How many times in your day do you wish you had more people and resources readily available to help you give your patients the complete care they need? Perhaps someone has behavioral health needs and you need assistance identifying available resources. Or a patient is having problems with their medication, or is experiencing transportation or housing issues that make it difficult to follow through on their care plan. Help is now just a phone call away.
LHP introduced the Care Resources Line as one of the Legacy Partner Solutions offerings earlier this year. This subscription service is now available to everyone in the LHP Network. You can consult with a pharmacist or care coordination specialist on any of your patients, whether they are a member of an LHP-contracted population or not.
Providers who sign up for the Care Resources Line receive a community resource directory that lists a variety of agencies and connections available to assist patients: placement and facilities, abuse/victim resources, mental/behavioral health services, transportation, veteran’s information, housing resources, food resources, support groups, shelters, medication resources, and aging and disability services.
One provider who has used the Care Resources Line had this to say: “It was nice not to have to do the footwork while you have 100 other things to do. The Care Resources Line staff was able to find a few other services we could not find on our own. It’s a great resource!”
