Legacy Medication Management Services now accepting referrals
Providers can now refer patients into Legacy Medication Management Services, a new service that is available to help patients with complex, critically important prescription medication regimens. Expanding on the anticoagulation clinics Legacy has operated for more than 30 years, this new service offers support for patients managing medications for hepatitis C, hypertension and/or hyperlipidemia, as well as polypharmacy patients.
This medication counseling service is intended to help achieve better outcomes for patients who have one or more of the qualifying conditions and are having trouble managing their medications. It also helps providers free up their schedule access by transferring this type of medication counseling appointments to pharmacists.
Legacy Medication Management Services has clinic locations at Legacy Emanuel Medical Center, Legacy Good Samaritan Medical Center, Legacy Meridian Park Medical Center, Legacy Mount Hood Medical Center, and Legacy Salmon Creek Medical Center.
