Complete one module by December 27, 2019 to receive a point towards your clinical integration score. You have 16 modules to choose from on a range of topics relevant to LHP providers. Be a part of LHP leadership – Applications for open pediatric board seat due May 3
If you’d like to help lead LHP in its next phase of growth, consider applying for a seat on the LHP Board of Managers. There is one seat open for an independent general pediatric physician. Applications will be reviewed by the LHP Board Development Committee in May, with board approval of the new member in June. The new board member will be invited to join the LHP Board in August 2019.

Have a performance measure to suggest?
We want to hear your ideas on performance measures we should consider for the future of LHP. Use this form to propose a measure. We’ll accept suggestions throughout the year, but those submitted by the end of April have the best chance of consideration for the 2020 program, as LHP’s leadership team is already at work planning for next year.
Get help with your Patient-Centered Medical Home certification
Certification as a Patient-Centered Medical Home is a membership requirement for primary care practices in the Legacy Health Partners network. If your practice is not yet certified as a medical home, LHP may have some resources to help with it. Legacy Partner Solutions for Patient-Centered Medical Home is a service offering to help LHP members with medical home certification. Grants to pay for this service may be available to smaller practices through Legacy Health Partners. Read the full story to learn more.
Quality Corner – Resources for treating AFib
The “Don’t Miss a Beat: All Things AFib” Clinical Collaboration Series event was an opportunity for LHP members to hear from three LHP cardiologists about guidelines for the overall management of atrial fibrillation (AFib), as well as updates on current treatment approaches, emphasizing anti-coagulation and alternatives for anti-coagulation. LHP practice administrators are invited to learn more about LHP’s contracting strategy and goals and provide input as the network considers how to structure future risk-based arrangements. Choose from these four informational meetings:
- April 18, noon-1 p.m., Legacy Emanuel Medical Center (Medical Office Building 2, MOB West)
- April 23, noon-1 p.m., Legacy Salmon Creek Medical Center (Conference Room E)
- May 2, noon-1 p.m., Legacy Mount Hood Medical Center (Boardroom)
- May 15, noon-1 p.m., Legacy Meridian Park Medical Center (Cafeteria Conference Room)
