Help available at no cost for LHP attributed patients struggling with chronic conditions
Three years ago, Legacy Health introduced Care Support Resources (CSR), a care management program that provides personal support for people who are struggling to manage chronic conditions such as asthma, diabetes, chronic obstructive pulmonary disorder, congestive heart failure, and coronary artery disease. By any measure this program has been a success. To date, approximately 825 patients have participated in the program, working with a nurse case manager, health coach, or pharmacist to set health goals and work on a plan to become healthier.
Patients who are members of LHP contracted populations may be identified to participate in Care Support Resources if they have one or more of the qualifying conditions and their claims data and health records show that they are at rising risk. When one of your patients is identified as a candidate for CSR, clinical members of the CSR team will contact your practice to discuss the integration of care. If you agree your patient could benefit from this program, a member of the CSR team will contact and invite the patient to join the program.
However, as a member of LHP, you can also identify your own patients to participate in Care Support Resources. A referral form is available here. If the patient is a member of an LHP contracted population and has one of the qualifying conditions, there will be no cost to the patient or your practice.
