Minimum clinical integration score is 60 percent for 2019
The Legacy Health Partners Board has approved a clinical integration (CI) score threshold of 60 percent that LHP members must meet to be eligible for any CI incentive for the 2019 performance year. This is consistent with the 2018 threshold. The minimum threshold supports network goals for provider engagement and high performance.
Any LHP provider who does not meet the minimum threshold will not earn a CI incentive for 2019 and will be placed on a monitoring plan for 2020. LHP providers who do not meet the threshold for two consecutive years will be terminated from the network. There is a process for requesting readmission to the network.
How can you ensure you meet the 60 percent requirement?
It is important that providers complete their annual Clinical Integration Education and a learning module, and that practice administrators attest to all applicable practice-level measures. But performance on quality measures is also vitally important to LHP’s growth and success. For example:
Fifteen primary care measures are reported at the individual provider level in 2019, so primary care providers have direct control over their score for several measures.
Other measures in LHP’s CI program continue to be measured at the network level. Review the data on your LHP CI program dashboard in the LHP Reporting Suite and think about how you can impact performance for measures that apply to you and your patients.
