Reminders and Announcements

The 2017 Clinical Integration (CI) education and Learning modules are now available. New this year: CME credit is available for completing the CI education, which covers LHP’s 2017 contracts, this year’s performance measures, dashboard enhancements and new provider-support services. Read more here.

Suggest measures for the LHP 2018 Clinical Integration program. Your proposed clinical measure may be either a process or outcome measure. Process measures typically record that an event has occurred (patient had breast cancer screening); an outcome measure typically records a final patient result or clinical result. Fill out the Clinical Integration Measure Suggestion Form by April 30.

Mark your calendar for an LHP MACRA Forum on June 19, 6-8 p.m. at Emanuel. This Legacy-hosted event is designed to assist members in planning for MACRA participation. Look for your invitation with registration details next week.

Practice administrators, watch for two surveys. The 2017 Access and Engagement survey is one of this year’s Clinical Integration practice performance measures. You answers will give LHP leaders a picture of our network’s accessibility for patients and will help LHP measure our effectiveness in engaging our members. The second survey will help us better understand the health plan benefits most important to your providers, and influence future product planning for our members. More information and links to the surveys will be in LHP’s Hot Topic next Thursday.

New services for LHP members participating in MACRA

Beginning this month, Legacy is offering three new services for LHP members planning to participate in MACRA. These services are designed to provide affordable assistance to make MACRA participation easier and help prepare LHP members for success in the new quality payment program. They are the first of several new MACRA services Legacy will introduce over the next year. Read the whole story.

Help shape LHP’s future: Apply for a seat on the Quality & Membership Committee

The provider-led LHP Board and four committees are comprised of 80 people deeply committed to the success of our network and its members. The LHP Quality and Membership Committee has an open seat. Members help provide guidance in the areas of performance measurement, quality improvement, clinical integration, peer review, membership requirements, credentialing and maintenance of network membership status. In order to maintain a balance of specialties across the committee, LHP is encouraging independent specialists to apply for this open seat. The committee meets monthly and members receive an annual stipend. The application deadline is May 1. Learn more and download the application here. Questions? Consider certification as a Patient-Centered Medical Home in 2017

For Primary Care practices considering how to invest for success in programs like MACRA and the Legacy Health Partners ACO contracts, certifying as a Patient-Centered Medical Home (PCMH) or increasing your certification level should be a priority. In addition to facilitating better coordination of care for patients, PCMH certification benefits practices participating in value-based care arrangements with government and commercial payors.
