QUALITY CORNER: Care gap reports now available

Have you looked at your LHP Dashboard lately? More performance values and targets are displayed and LHP’s wellness and chronic condition care measures reflect data for claims paid through August 30, 2017. The LHP Network is meeting or exceeding performance for some measures, but we are not on track to meet our performance goals for others. Providers can now generate care gap reports from their LHP Dashboard for eight wellness and chronic condition measures to ensure that LHP attributed patients get appropriate and timely care and screenings. Interested in leadership? Apply for a seat on an LHP committee

It’s an exciting time in LHP’s development. If you’d like to help lead LHP in its next phase of growth, apply for a seat on one of the four committees. Applications are due November 1. The time commitment is four to six hours a month and members are compensated with a stipend. Read more about each committee and download an application.

Deadlines approaching for 2017 MACRA participation

For practices participating in MACRA for 2017, we are within the final 90-day window to report data for the Quality, Advancing Care Information and Improvement Activities categories for the year. However, for the Quality category, you still have the option of reporting a shorter period of data – as little as one day – before the end of the year to avoid receiving a negative payment adjustment in 2019. Advocating on your behalf

The Centers for Medicare & Medicaid Services recently solicited stakeholder opinions about the documentation requirements associated with the different Evaluation and Management levels of service. 2017 financial incentive methodology

There are a few changes this year to how providers will be rewarded for their performance on this year’s clinical integration program. The most significant changes benefit PCPs. Final details will be approved in the spring by the provider-led Finance and Contracting Committee and Board. Read the Pay For Value document saved on the LHP team site.
