Legacy Health’s Response to APP Strike Notification

Legacy Health has received an open-ended strike notice from the Oregon Nurses Association, which represents approximately 140 of our Adult Specialty Advanced Practice Providers. 

The strike is slated to begin on Dec. 2 at 6 a.m. outside Legacy Emanuel Medical Center and Legacy Good Samaritan Medical Center. 

Now that we have received a strike notice, bargaining will be paused during the 10 days leading up to and throughout the duration of the strike. While we remain committed to bargaining in good faith with the ONA, we are now focused on the care of our patients and will continue to allocate our available resources to meet this goal.  

Legacy Health has bargained in good faith in over 30 sessions in the last 18 months with this bargaining unit. Our philosophy throughout this process has been to reach a fair and market-aligned agreement that supports our employees and the long-term stability of our organization.  

Legacy Health has offered an average 10% pay increase for all adult specialty advanced practice providers. Our compensation offer is based on local and national data. It is essential to look at the APP market holistically. Through exhaustive regional benchmarking we have identified dozens of examples of systems posting APP positions with compensation that is lower, and in some cases, significantly lower than our offer. 

Our compensation model is based on market data, internal equity and what is financially sustainable for our organization given our unique financial challenges. Because our health system financial ecosystems are different, the pay structures are not directly comparable.  

We understand employees may compare our compensation to other local systems. Each health system operates within its own financial and economic environment. Legacy’s realities are not the same as others. As a safety-net health system, our mission is to care for everyone in our community, including those who rely on Medicaid and other governmental programs. These payers represent a disproportionate share of our patient population, and governmental reimbursement does not cover the full cost of providing care.  

This reality puts sustained pressure on our finances. We must balance fair and competitive compensation with the financial constraints required to keep our organization viable – especially during a time when we continue to run unsustainable operating losses.  

It also influences the financial parameters we must consider at the bargaining table. Until reimbursement structures reflect the true cost of care, there are limits to what any organization can sustainably commit to within a single contract. Having said that, Legacy is proud to offer market-competitive wages. 

We want to reaffirm with our patients and the community that we have activated contingency plans to ensure continuity of operations and care for our patients. We will notify patients should anything change with a scheduled appointment.
