Quality Corner: Remote Blood Pressure Monitoring Pilot

Clinical Pharmacy-Led Hypertension Management

Legacy’s clinical pharmacy team recently launched a pilot program utilizing Remote Physiologic Monitoring (RPM) to support longitudinal blood pressure (BP) management in primary care patients at Legacy Clinics. The program leverages Bluetooth-enabled BP devices and pharmacist-led interventions under collaborative practice agreements.

Beyond the known clinical impact on cardiovascular morbidity and mortality, this program matters because improved BP control directly supports our performance in value-based care with a significant impact on performance metrics. Expanding this program can help us meet quality targets while enhancing patient outcomes. Click here to read more about the pilot structure, early results, and feedback.



Specialty Spotlight: Podiatry specialists in LHP & the importance of protective footwear inside the home

As we spotlight podiatry and diabetes care this August and September, we’re reminded of a simple yet powerful truth: footwear matters… especially indoors. According to Elliot Michael, DPM, a recent study conducted at the Hillsboro Foot and Ankle Clinic revealed a compelling link between indoor footwear habits and diabetic foot ulcer outcomes. Among diabetic patients who developed or struggled to heal ulcers:
  • One-third reported not wearing shoes or hard-soled footwear at home.
  • 86% of those who healed wore shoes at home.
  • 57% of those with non-healing ulcers did not wear shoes indoors.

Important information for practices as you credential new providers with Humana

Be sure to update CAQH for any new PCP providers you add to LHP

We’d like to remind practices as you submit new PCP or Pediatric providers for inclusion in the Legacy Health Partners network, please ensure their CAQH (Council for Affordable Quality Healthcare) profiles are fully updated to meet Humana’s credentialing requirements. This helps prevent claim denials and ensures timely patient care. Humana provider network effective dates are 30 calendar days after all required documentation is received in the CAQH. As a reminder, LHP sends provider rosters to all our payors once at the end of every month.

Ending respiratory testing requirements at Unity Center

As of Aug. 26, individuals being referred to Unity Center for Behavioral Health will no longer require the respiratory PCR test as part of routine medical clearance. Instead, hospitals or providers referring patients will need to utilize symptom-based screening. Testing will continue to be required for patients who are symptomatic and should be considered for patients who have recent exposure or are unable to provide a reliable medical history. Legacy Health Partners (LHP) is excited to announce a new partnership with Health 2 Business (H2B), marking our first steps into direct-to-employer, non–value-based contracts. These arrangements are designed to offer flexible, cost-effective healthcare solutions for self-funded employers in Oregon and Southwest Washington, where LHP serves as the exclusive provider network.   • For primary care: Be sure to review the Member Attribution report in Power BI for a list of the LHP patients attributed to you. If you believe a member is attributed to you in error, download the Reattribution Request Form from the LHP Team Site and include the patient’s correct PCP and email the form to LegacyHealthPartners@lhs.org before Sept. 30.

  • A correction to our July newsletter issue. We inadvertently omitted one of our valued dermatology practices from the listed LHP network providers. We have corrected the practice list in the online version of the article and would like to acknowledge and welcome Michele Thompson, MD Dermatology to the Legacy Health Partners network.
