Quality Corner: Elevating quality results this quarter

As we move into the fall, we are excited to spotlight our current quality priorities, invite your participation in our annual Quality Awards, and announce engaging opportunities to connect and collaborate with our Quality & Performance Improvement (QPI) team. We are inspired by the dedication and innovation shown across our network and encourage every LHP member to take part in these initiatives. We can’t wait to collaborate with you this fall!

LHP Quality Focus Areas: Strengths and Opportunities

This year, LHP is focused on several key quality and utilization rates to ensure we’re driving meaningful results for our patients. Recent performance check-ins have revealed important strengths and opportunities:

• Strengths: We are trending ahead in diabetes eye exams, controlling high blood pressure, and statin therapy for patients with both diabetes and cardiovascular disease.
• Opportunities: Areas like breast cancer screening, cervical cancer screening, and A1c testing have room for additional focus. Additionally, hospital follow-ups, pediatric ED visits, and outreach to our unattributed population present further opportunities for improvement and education.
• Collaboration with the Children’s Health Alliance, quarterly regional quality/utilization meetings, and focused initiatives, such as a new hypertension toolkit, are ongoing efforts to meet these quality goals and improve care coordination across the network.

Annual LHP Quality Awards: Celebrate Your Team’s Impact!

We invite all LHP members to participate in our annual LHP Quality Awards. These awards recognize improvement projects that enhance patient care, clinic operations, and overall health outcomes.

• Eligibility: All independent and employed LHP practices are welcome to apply, including primary care, specialty, and inpatient teams.
• Benefits: Winning submissions receive financial awards (1st Place: $2,000, 2nd Place: $1,500, 3rd Place: $1,000), certificates, and public recognition through newsletters and clinic visits with treats for staff. Sharing successful strategies helps boost morale, staff engagement, and encourages replication of best practices across our network.
• Application Deadline: October 31

Regional Quality Meetings: Connect, Learn, and Advance Quality

This October, LHP’s Quality & Performance Improvement (QPI) staff will be hitting the road for a series of regional meetings designed to bring care teams together for learning, discussion, and action. Whether you’re part of primary or specialty care, these meetings are relevant to all teams who share the goal of elevating quality across our network.

• Topics: Sessions will focus on optimizing HCC recapture, advancing performance in key quality areas, and sharing strategies for early capture of quality data to meet payer deadlines.
• Audience: All LHP care teams (primary, specialty, and inpatient) are encouraged to attend and participate.
• Collaboration: These meetings are a valuable opportunity to exchange ideas, address challenges, and engage with QPI staff and peers in a supportive and collaborative environment.
