Clinical Integration Program incentives for 2024 coming soon

As we approach the end of October, we are excited to announce that final financial results are in for 2024, and our clinically integrated network collectively earned $4.2 Million in shared savings and quality earnings!

This past year marked a significant evolution in the Clinical Integration (CI) Program’s incentive model. We heard your feedback and recognized the need for an incentive model that is transparent, meaningful, and better aligned with the realities of clinical practice. Last year, LHP’s Finance and Contracting committee and Board of Managers approved a revised incentive model with these goals in mind.

Incentive payments for the 2024 performance year will soon be distributed to practices, so we want to remind you of the key improvements in the revised incentive model:

  • Exceptional performance will be rewarded! Previously, providers were rewarded only if they met the measure target, now the higher their percentile, the more they earn.
  • CI Program funds and Shared Savings distribution are calculated separately.
    • CI Program Funds earned by each provider will be a sum of quality, practice contract participation, and citizenships measures.
    • Shared Savings will be distributed based on a provider’s quality score.
  • Practices will receive points for applicable contracts they participate in.
  • Practices and providers will still earn incentive for citizenship measures and attribution tiers.
  • Incentive eligibility is still contingent on completion of the annual CI Education video.
  • Hospital pool funds are distributed in accordance with achievement of hospital measures.
  • Practices are only eligible for care coordination and Shared Savings Funds from contracts they participate in. Program incentives are distributed annually to all eligible practices and care coordination/per-member per-month (PMPM) funds are distributed to PCPs quarterly.

We believe the improvements to the Clinical Integration Program’s incentive model for 2024 represent a significant step forward in fostering a collaborative, quality-driven network. Your dedication to patient care is what makes this progress possible, and we are committed to rewarding that dedication in meaningful ways.

We’re looking for leaders who care deeply about delivering value, improving health, and bring experience in areas like quality, cost containment, contracting, and strategy. We welcome applicants from all backgrounds and experiences.

Board and committee positions receive an annual stipend for their service and meetings are held virtually.

Network Spotlight: A Decade of Legacy Health Partners

This year marks an extraordinary milestone: Legacy Health Partners (LHP) is celebrating its 10th anniversary! Over the past decade, our network has grown in size and impact, united by a shared commitment to collaborative care, patient-centered excellence, and continuous innovation. Providers, clinics, and hospitals have worked hand-in-hand to ensure seamless care for our patients. Our network has grown to include new specialties, additional care sites, and more providers, which has allowed us to reach more patients. By sharing best practices, coordinating resources, and learning from each other, we have elevated the standard of healthcare across our entire network and made progress towards value over volume.

As we celebrate this 10-year milestone, we invite all LHP members to share what Legacy Health Partners means to them. Reflect on the difference we have made together, the lives improved, the connections made, the challenges overcome, and the moments you are most proud of. Breast Cancer Awareness Month is underway

Breast Cancer Awareness Month is the month-long reminder for every woman to take proactive steps in the fight against breast cancer. Routine mammogram screenings starting at age 40 remain one of the most effective tools in treating breast cancer.

For those at higher risk of getting breast cancer, additional prevention measures — such as getting a formal risk assessment or even participating in genetic testing — may be recommended. These services are available through Legacy Health’s High Risk and Genetics Clinic.

For Amie Bates, PA-C, a lead provider at Legacy Health’s High Risk and Genetics Clinic, every woman should consider having a formal risk assessment completed by age 30. Learn more.

  • Last week, Legacy Health announced service changes impacting programs and clinics across Legacy Emanuel Medical Center, Legacy Good Samaritan Medical Center, Legacy Salmon Creek Medical Center, and Legacy-GoHealth Urgent Care. Read more here.
  • LHP offers two learning activities that are due on December 19. LHP has been sending email reminders directly to providers so you know if you have not yet completed these activities yet.
