Imaging services are also provided at the main hospital (19300 SW 65th Ave). Please check carefully the location address for your appointment.
Where to park:
Our parking lot is located in front of the Medical Plaza Building 2.
Where to check-in:
Please check in at the front desk located in suite 165 of Medical Plaza Building 2.
What to bring:
Please bring your insurance card, photo ID, and any paperwork that your physician provides to the appointment. If you are having an exam that requires a contrast, please bring a list of the medications you are currently taking.
Services can only be performed with an order from a physician. Walk-ins are not available.

Accreditation
Legacy Meridian Park is accredited by The Joint Commission and the American College of Radiology (ACR) in CT, MRI, Ultrasound and Mammography. The Breast Health Center was named as a Breast Center of Excellence by the ACR, and has also received accreditations from the National Accreditation Program for Breast Centers (NAPBC) and Mammography Quality Standards Act (MQSA).
Medicare patients, please note: Some of our clinics are considered "hospital-based" because they share the same licensure with a Legacy hospital. The services or procedures you receive at Legacy's hospital-based clinics may include a facility charge in addition to a physician charge.
