Dr. De Olazo is board-certified in internal medicine. She earned her Doctor of Medicine and Surgery from the University of Santo Tomas in the Philippines and completed her internship and residency at Forum Health Hospital in Ohio. With over 15 years of experience, Dr. De Olazo has a deep passion for primary care — building meaningful, long-term relationships with her patients while delivering comprehensive, evidence-based care. She emphasizes preventive medicine and takes a collaborative, personalized approach to help each patient achieve their best health. Over the course of her career, Dr. De Olazo has taken on clinical and executive leadership roles, including serving as Site Medical Director and later in senior care and long-term care administration — experiences that deepened her understanding of whole-person, continuum-of-care medicine. These roles strengthened her commitment to patient dignity, quality outcomes, and compassionate care across all stages of life.
